Here at Middlebury Sport & Spine, we value our patients. We acknowledge that going to a new place for the first time can be nerve racking. We want to do everything we can to eliminate any unnecessary stressors, which is why we include the option of printing and completing your forms at home. We think of our patients as part of our extended family, and we are excited to meet you!

Treatment

If your condition and specific circumstances allow, you may receive your first treatment at the conclusion of the initial appointment. This may include spinal adjustments, physical therapy modalities, and/or soft tissue therapies.

Insurance

Most insurance plans are accepted at our office. We participate with Blue Cross & Blue Shield, Cigna, CBA Blue, Medicare, and in some instances auto accident, workers compensation and personal injury insurances. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. As a courtesy our staff will make every attempt to verify and explain your benefits before treatment, at no charge. Please be aware this not a guarantee of coverage. Final liability, if any, can not be determined until the insuarnce company has received and properly adjudicated the claim.

No Insurance

If you do not have insurance there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable. We also offer access to ChiroHealthUSA (CHUSA). CHUSA is a discount medical plan that allows us to offer discounted fees for our services. This is not an insurance plan or a qualified health plan under the Affordable Care Act.
